New fee structure:
Elementary technology fee - $55
Middle and High School technology fee - $75, plus class fee (listed below):
6th grade class fee - $60
7th grade class fee - $60
8th grade class fee - $60
Freshman class fee - $45
Sophomore class fee - $60
Junior class fee - $75
Senior class fee - $90
Sports Particapation Fee - $50 per athlete and per sport
- Students new to the district should report to their respective schools on Friday, August 3 from 8 a.m. to 3 p.m. NEW STUDENT ENROLLMENT would include kindergarteners who did not attend the Kindergarten Enrollment in April. Parents of new students to USD 204 that are concerned about which elementary school their students should attend are advised to contact the USD 204 Transportation Department at 913.441.2493.
The Free and Reduced Application for meals and textbooks will be mailed to parents of returning students during the month of July from the Central Office.

What should I bring to enrollment as a new student?
Preschool, kindergarten and new to the district first-graders: (By Kansas law, children must be five years old on or before August 31 to enter kindergarten)
Birth Certificate: A certified state copy of your child's birth certificate as per Kansas statute 72-53,106(b)
Parents may apply for a birth certificate for their child by visiting the Kansas Department of Health and Environment Web page and downloading the appropriate form. (Visit the department's site at www.kdhe.state.ks.us/vital/birth.html for a link to PDF forms.)
Immunization Records: A copy of your child's immunization records or written validation of medical or religious exemption (see Immunization Requirements at left).
Proof of Residency: A mortgage stub, lease agreement, or utility bill with the name of the parent/guardian on it is required. Your home address must be within the USD 204 school district.
Affidavit Regarding Custody and Identity of Student: Students enrolling for the first time (preschool, kindergarten and students new to the District) must complete an Affidavit Regarding Custody and Identity of Student. This form must be notarized and its accompanying documents must be turned in to the school before the student can be enrolled. (see "AffReCustody of Student" form)
Social Security Number: A copy of your child's social security card.
Physical Exam: Preschoolers and kindergartners are required to have a physical exam in accordance with Kansas Statute 72-5214 (see "Physician's Health Assessment" form).
New to the district students entering second-through sixth-grade:
Proof of identity (one of the following):
A certified state copy of your child's birth certificate as per Kansas statute 72-53,106(b)
Certified transcript of school records
Actual school records from the sending school
Original hospital certificate of birth

The annual physical must be obtained after May 1 for use in the following school year.
All items on the form must be completed.
A parent's signature is required for student participation.
A new physical is required each school year for sports participation.
